Use of Letermovir for CMV Prophylaxis after Allogeneic Hematopoietic Stem Cell Transplantation: Review of the Literature and Single-Center Real-Life Experience

چکیده
Cytomegalovirus (CMV) reactivation after allogeneic hematopoietic stem cell transplant (allo-HSCT) is mainly due to an increase of latent viremia in previously exposed patients. Furthermore, CMV this setting has a significant impact on patient survival. Traditional approach post allo-HSCT was pre-emptive treatment with antivirals the case increased viremia. However, since 2017, new antiviral compound, letermovir, been introduced clinical practice and deeply changing common approach. The toxicity profile letermovir allowed its use prophylaxes patients at high risk reactivation. This review will focus present role discuss some possible future applications drug. Finally, our single center management view recent introduction be discussed.

Treatment of CMV infection after allogeneic hematopoietic stem cell transplantation.
Despite a remarkable reduction in the past decades, cytomegalovirus (CMV) disease in allogeneic hematopoietic stem cell transplant (HSCT) recipients remains a feared complication, still associated with significant morbidity and mortality. Today, first line treatment of CMV infection/reactivation is still based on dated antiviral compounds Ganciclovir (GCV), Foscarnet (FOS) and Cidofovir (CDF) w...

Cytomegalovirus (CMV) genotype in allogeneic hematopoietic stem cell transplantation
BACKGROUND Based on sequence variation in the UL55 gene that encodes glycoprotein B (gB), human cytomegalovirus (CMV) can be classified into four gB genotypes. Previous studies have suggested an association between CMV gB genotype and clinical outcome in patients who underwent an allogeneic hematopoietic stem cell transplant (HSCT). The goals of this study were identify patients with active inf...
متن کاملPulmonary Histoplasma Infection After Allogeneic Hematopoietic Stem Cell Transplantation: Case Report and Review of the Literature
Histoplasmosis causes a wide spectrum of clinical illness, including disseminated infection in the immunocompromised. We report a case of pulmonary histoplasmosis in an allogeneic stem cell transplant recipient and review the literature on this topic. Histoplasmosis in this patient population is uncommon, but it is associated with poor outcome.
